3 Easy Steps to Complete Your Health Checkup
- Register online in less than 2 minutes. All we need are a few pieces of information. You can schedule online by using the QR code on the flyer or by using this link Schedule Your Appointment
- Complete your onsite checkup in under 20 minutes.
- Review your results with a certified Nurse Practitioner and receive personalized recommendations on how you can improve or maintain your health.
Those who complete their checkup before 09.30.24 are eligible for the following incentive:
Participants who complete a yearly checkup will earn a premium credit of $120 ($10 per month) beginning in January 2025

If you are unable to attend the clinic, Click on the link below for a PCP form that you can use at your regular physician's office. Have your primary care physician complete the form and then send it to Catapult using the directions on the form. The deadline to submit your forms to Catapult is 09.30.24.
Also attached is an exemption form should you have a condition that exempts you from participating in the clinic. Those exemptions are listed on the Exemption Form.
